A Family Health Team is an approach to primary health care that brings together different health care providers, like doctors, nurses, pharmacists and social workers who work together to provide the highest possible quality of care for you, the patient. The different health care professionals in family health teams, work together and use their own experience and skills so that you receive the best care, when you need it, and as close to home as possible.
Family Health Teams improve access to health care across Ontario to keep people healthy and improve chronic disease prevention and management.
